Breast explant surgery removes breast implants previously placed for cosmetic enhancement or reconstructive purposes. Explants have gained increasing attention in recent years as more individuals seek to address implant concerns or complications. In fact, industry data shows that about 219,000 women had breast implants removed in 2021. 

Breast explant surgery can be needed for several reasons, including implant-related health issues, such as Breast Implant Illness (BII), rupture, or leakage, or due to personal changes in cosmetic preferences.

Understanding Breast Explant Surgery

Unlike breast implant surgery, explant surgery focuses on removing existing implants and, in some cases, the surrounding scar tissue capsule. 

Breast explant surgery can be performed using various techniques, including simple implant removal, total capsulectomy, or en bloc capsulectomy, depending on the patient's needs and the surgeon's recommendation.

While implant surgery adds volume to the breast, explant surgery leads to breast reduction and may need additional procedures to correct changes in tissue and skin elasticity. 

“Sometimes, it’s also performed alongside a breast lift or fat transfer for better aesthetic results,” explains Dr. Usal. 

Post-Breast Implant Removal: Medical Signs You May Need Breast Explant Surgery

If you’ve had breast implant surgery, there are several health-related signs that may indicate that post-breast implant removal may be best, including:

  • Breathing or sleeping problems that developed post-surgery
  • Chronic fatigue, headaches, or unexplained systemic symptoms that could be related to BII
  • Distortion or changes in breast shape
  • Hardening of the breast tissue, which may indicate capsular contracture, a condition in which the scar tissue around the implant tightens.
  • Pain or discomfort in the affected breast
  • Persistent joint and muscle pain without other apparent causes
  • Pressure sensation on the breast
  • Sudden deflation or change in breast size, which may suggest implant rupture
  • The ability to see or feel the outline of the implant through the skin
  • Upward movement of nipples while the implant falls lower, indicating implant bottoming out
  • Visible or palpable changes in implant position, such as bottoming out or implant displacement

  • Aesthetic concerns, including dissatisfaction with breast size or appearance, changes in breast shape due to aging, pregnancy, or weight fluctuations, or a desire to return to a more natural look
  • Lifestyle changes, such as the implants interfering with physical activities
  • Practical concerns, including the need for an exchange or replacement due to the age of the implants 

What to Expect During a Consultation 

During a breast explant surgery consultation, patients generally undergo a thorough evaluation and discussion with the surgeon. This includes a review of medical history, a physical examination, a discussion of reasons for the surgery, and an explanation of surgical options. The surgeon will also educate the patient on the potential risks and benefits of breast implant removal and any pre-operative instructions. 

“This comprehensive approach ensures that patients are well-informed and prepared for their breast explant surgery,” says Dr. Usal. 

Breast Implant Removal Recovery and Aftercare Tips  

Following a breast explant surgery procedure, you’ll want to be sure to follow the specific recovery and aftercare instructions set by your surgeon. While these can vary from person to person, the following tips are generally given to ensure optimal healing: 

  • Attend all follow-up appointments with your surgeon to monitor your progress.
  • Avoid strenuous physical activity or heavy lifting for at least 3-4 weeks after surgery.
  • Be patient with the healing process, as full recovery can take 6-12 months.
  • Get plenty of rest, especially in the first 1-2 weeks after surgery. 
  • Take time off work and limit physical activities.
  • Keep incisions clean and dry to prevent infection.
  • To support the healing process, maintain a healthy diet rich in fruits, vegetables, whole grains, and lean proteins.
  • Report any unusual symptoms or complications to your surgeon immediately.
  • Sleep on your back with your upper body slightly elevated for the first 3-4 weeks. Using a recliner can be helpful.
  • Stay hydrated by drinking plenty of water.
  • Take light walks as soon as possible to promote blood circulation and aid healing.
  • Wear the recommended surgical compression bra or garment as instructed, typically for at least 4 weeks. This helps reduce swelling and supports the breasts as they heal.

“Remember, everyone's recovery is unique, so it's crucial to follow the specific instructions provided by your surgeon and communicate any concerns you may have during the recovery period,” says Dr. Usal.
