Responsibilities

  • Coordinates individualized personal assistance for the plan of care of all breast cancer patients
  • Coordinates care by referring patients to medical oncology, radiation oncology, genetic counselor, or genetic testing
  • Provides quality care and answers patient questions and meets the needs of the cancer patient
  • Communicates with all members of the healthcare team, as appropriate, about patient and family needs and/or concerns
  • Meets with oncology nurses, at multiple facilities, whenever needed or requested to do so.
  • Responds to patients requests for information regarding the disease process, expected side effects of treatment and offers community resources
  • Provides personal guidance to patients as they move through the healthcare system for treatments or plans
  • Provides patients information or referrals to finding doctors or treatment facilities whenever needed.
  • Helps to make appointments with MRI’s and other related breast studies, making sure that authorizations are completed by scheduler for appropriate testing
  • Sends out tumors/biopsies for onco-type testing after “in office” procedures and follows thru with pathologies and results
  • Attend weekly tumor boards according to physician’s schedule, at multiple facilities
  • “Mammoprinting” tumors if needed
  • Sets up prehab and rehab for breast cancer patients
  • Genetic testing referrals and keeps track of genetic testing patients for tumor boards
  • Follows up with patients after surgery or referrals to oncology to monitor progress of disease and patient's response.
  • Demonstrates a commitment of team work and cooperation, assisting other staff members whenever needed
  • Assignment of current LPN/CMA duties, as previously agreed not limited to medical clearances, PATs, and follow up of results.

Requirements

  • Current NJ LPN/CMA license

  • Breast Patient Navigator Certification

  • Current CPR/BLS

  • At least 1 year breast oncology experience

  • Office experience preferred. EMR knowledge is a plus, training available using Nextech
